Tier 3 Eligibility Criteria:
✅ Age 18+
✅ Not pregnant or breastfeeding
✅ No active eating disorder
✅ No bariatric surgery in the last 12 months
✅ Not attended Tier 3 in the last 2 years (from discharge date)
✅ Completed a Tier 2 weight management programme, such as:
🔹 MyWeightMatters
🔹 National Diabetes Prevention Programme (NDPP)
🔹 National Digital Weight Management Programme
🔹 Freshwell Low Carb Programme
🔹 Slimming World
🔹 Weight Watchers
AND meet one of the following health criteria:
⚖️ BMI of 40+ plus at least one of:
Type 2 Diabetes
Obstructive Sleep Apnoea
Severe Asthma
Cor Pulmonale
Obesity Hypoventilation Syndrome
Currently using CPAP
Ischaemic Heart Disease
Heart Failure
QRISK score over 20%
Subfertility
Severe Acid Reflux, Oesophagitis or Barrett’s Oesophagus
