Total Triage – GP Appointments (excluding Nurse’s, HCA and Phlebotomist appointment)

We are now operating total patient triage for GP appointments. If you need a GP appointment or have a medical issue, complete the simple form online which can be found under appointments then complete a triage form.
Nurse, HCA and phlebotomist appointment do not require you to complete a triage form. These can be booked by calling the surgery on 01787 222022.
